The program gives the patient respiratory clinical support during the critical 30-day post-discharge period through multiple contacts with a respiratory therapist in the home and on the phone. This ensures that the patient is maintaining their treatment plan and progressing toward their goals. Patients are also evaluated with overnight oximetry and physicians are quickly alerted to any clinical concerns and actions by the respiratory staff.
